Manual therapy and fascial release are hands-on treatment techniques used by physical therapists to relieve pain, reduce muscle tension, and improve movement. Through gentle joint mobilizations, soft tissue work, and targeted pressure on tight or restricted areas, these techniques help release tension in the muscles and fascia - the connective tissue that surrounds and supports your muscles.
Manual therapy includes techniques such as joint mobilization, soft tissue manipulation, and guided stretching to improve joint movement and relieve muscle tightness. Fascial release specifically targets restrictions within the fascia, which can become tight or stiff due to injury, poor posture, repetitive strain, or prolonged inactivity. When fascia becomes restricted, it can contribute to pain, stiffness, and limited movement in different parts of the body.
By restoring normal tissue mobility and improving circulation, manual therapy and fascial release can reduce stiffness, enhance flexibility, and support faster recovery from injury. These treatments are often combined with therapeutic exercises to improve movement, prevent recurring pain, and promote long-term musculoskeletal health.
Moderate evidence indicated that manual therapy increased local pressure pain thresholds in musculoskeletal pain, immediately following the intervention. (1)
Currently, manual therapy is used in the treatment of patients with various disorders including extremityjoint dysfunction, spinal dysfunction, lymphadema, TMJ disorders,headaches, cystic fibrosis, nerve entrapments, and following immobilization. (2)
Spinal manipulation/mobilization is effective in adults for: acute, subacute, and chronic low back pain; migraine and cervicogenic headache; cervicogenic dizziness; manipulation/mobilization is effective for several extremity joint conditions; and thoracic manipulation/mobilization is effective for acute/subacute neck pain. (3)
Fascia manipulation exerts its therapeutic effects through a combination of anti-inflammatory cytokine modulation, mechanoreceptor stimulation, and improvement in fascial gliding and proprioceptive feedback. These mechanisms collectively support its role in pain relief and neuromuscular function. (4)
